 the payment will be credited back to the credit card.

 

For new applications:

 Do not submit a payment unless a policy # has been issued.

A policy # is not issued until the application has been reviewed by an underwriter.

 

AGENTS: SUBMIT NET AMOUNT ONLY.

 

 If you are making a payment for a continuation of coverage

 you must still provide the signed continuation of coverage form.

Signed form can be faxed to (770) 717-8620 with a note

 stating that payment was made via credit card

(include your transaction # on the faxed form)

 

Please note that one payment can be made for multiple policies.

  There is a $5.00 processing fee per credit card transaction.
