Enter premium amount from quote to know exactly what amount to collect from your insured and exact amount to submit to GUA

STEP FOUR

Complete the appropriate application. A separate application is necessary for each building. Every line on application must be completed. Sign the application, submit with photo of front and back of property, a copy of your on-line quote and completed agent premium worksheet. Payment must be received with the application, personal checks are not accepted. Keep a copy of everything that you submit for your own records.
