   Enter premium amount from quote to know

STEP FOUR

   Complete the appropriate application. A separate

   application is necessary for each building. Sign the

   application, submit with photo of front and back

   of property, a copy of your on-line quote and

   completed agent premium worksheet. Payment

   must be received with the application; personal

   checks are not accepted. Keep a copy of everything

   that you submit for your own records.
